Immunization Requirements for Child Care

Immunizations for Children
A Guide for Parents and Care Givers
These are the vaccinations every child needs!
At birth Hep. B
1-4 months Hep. B
2 months DTaP Hib Polio PCV7*
4 months DTaP Hib Polio PCV7*
6 months DTaP Hib Polio PCV7* Hep. B
12 months MMR Hib Chickenpox PCV7*
15 months DTaP
2 years Hep. A*

4-6 years DTaP Polio MMR
11-12 years Td (tetanus booster) Hep. B ** Chickenpox**
 
These are generally the earliest times children can get these shots. Some clinics use a slightly different schedule. Check with your clinic to make sure your child is immunized on time. Also, make sure you ask your clinic for a record of shot dates and take them with you to update your child's daycare and school record.
 
Hep. B:             protects against hepatitis B, a serious liver disease
DTaP:               protects against diphtheria, tetanus (lockjaw), and pertussis* (whooping cough)
Hib:                   protects against Haemophilus influenzae type b
Polio:                inactivated (injected) polio vaccine (IPV) protects against polio
PCV7*:             pneumococcal conjugate vaccine protects against serious pneumococcal infections
MMR:                protects against measles, mumps, and rubella (German measles)
Chickenpox:   varicella vaccine protects against chickenpox
Hep. A:             protects against hepatitis A, a liver disease more common inOregon
 
*PCV7, Hep. A and pertussis (whooping cough) vaccines are recommended, but are not required to attend daycare. Diphtheria and tetanus vaccines are required but can be given as DT (without pertussis).
 
** Hep. B is only needed at 11-12 years if the child has not already received the vaccine series. Chickenpox is only needed if the child has not already had the vaccine or chickenpox disease.

Immunization Requirements for
Registered Child Care Homes
 
 
 
1) Every child (including your own children) needs to have a Certificate of Immunization Status (CIS) form completed and signed by the parent.
 
 
2) You can get these forms from your local county health department.
 
 
3) The form should show at least one dose of each vaccine right for the child's age (see reverse side for details).
 
 
4) A parent can choose to sign a religious exemption. This cannot be used as a reason not to care for the child.
 
 
5) Some children cannot be immunized because of a medical problem. Their parent should give you a note from the doctor or a county health department. Staple the note to the CIS.
 
 
6) These forms need to be updated by the parent often. (See reverse side for ages when children need shots.)
